MK4 output issues

My MK4 stopped putting out any signal on outputs other than 1/2. I'm on OSX 12.6. I've used the interface on that OS many times. I've tried updating firmware and the update fails every time. Progress bar always gets stuck. I don't know what else to try. Any advice?

I feel like I’ve seen an increase in outputs going silent. Toggling them off and back on in the grid usually fixes it here.
